Dr. Faber is a participating provider (in network) with several benefit carriers. We can generally submit for your benefits from any carrier other than HMO's. While we accept assignment of benefits, you will need to pay your estimated copayment at your visit. We will bill your insurance company as a courtesy and do our best to maximize your benefits.

Our office is located at the south west corner of El Adobe Plaza in San Juan Capistrano, at the intersection of Del Obispo and Camino Capistrano, next to the railroad tracks. Our office fronts Del Obispo street and is right across from Mollie's Restaurant.
